AI in healthcare is the most actively searched use case for modern transformer-based Large Language Models (LLMs). And when a new study starts pointing at “bias” and “accuracy gaps” in hospital AI tools, it is easy to assume the whole category is a minefield. But like most things in enterprise AI, the reality is more specific, more actionable.

A recent study out of the University of Minnesota School of Public Health looked at how hospitals are actually using AI-assisted predictive tools. Not in theory, but in production. These systems are already embedded in workflows, helping flag patient risk, prioritize care, and guide operational decisions.

The study highlights a familiar pattern: predictive models can vary significantly in accuracy across different patient populations. In some cases, tools designed to identify high-risk patients performed unevenly depending on demographic factors, introducing the potential for biased outcomes if left unchecked.

This is where the conversation usually drifts into “AI risk” at a philosophical level. But for CIOs and CTOs, the more useful framing is much narrower:

The Reality: Where Non-Medical AI Actually Shows Up

When people hear “AI in healthcare,” they tend to think scribes, imaging, or diagnostics and treatment recommendations. But most of the real-world AI deployments that exist in hospitals today sit outside of medical decision-making entirely.

They show up in:

  • Patient intake and scheduling prioritization
  • Resource allocation (beds, staffing, equipment)
  • Billing, coding, and claims processing
  • Operational forecasting and capacity planning
  • Internal knowledge retrieval and documentation workflows

These are not sexy new AI-medicine use cases. They are the corporate back-office operations that can be automated, that soak up hospital budget and handle the unglamorous work of making things run.

In most hospital environments, these predictive tools behave much more like an RPA layer than an decision-making agent. This means the real risk is not the model itself, but how that automation layer integrates cleanly over non-medical workflows. Too haphazardly, the model can run amok. But too brittle and it offers none of the upside over traditional RPAs of the past.

If a biased model is layered overtop of an existing function with no friction, that is a problem. But if the same model is one of several inputs, reviewed, contextualized, and occasionally overridden, the risk profile can be mitigated dramatically.

So this is not just a model problem. It’s about workflow design when bringing in new automation layer.

What the Study Signals

But even operationally there is still risk that “AI is biased.” The models’ bias becomes operational when it is embedded into automated workflows without visibility or control.

For technical leadership, that surfaces a few very practical questions:

  1. Where are AI outputs being consumed automatically?
  2. Do we have auditability at the workflow level?
  3. Can the system degrade gracefully?

Critical breakpoints to check in on a model significantly mitigates this risk. By implementing these control points, the model’s edge cases can be surfaced and remedied in much quicker fashion than you have with models working within patient-care.

The instinct is to evaluate models: Which one is more accurate? Which one is less biased? But in practice, the bigger gains (and the safer deployments) come from structuring the workflows around those models. AI outputs built around SOPs and workflow diagrams can manage the edge cases in an operational workflow. It is a much safer way to deploy LLMs than in medical-facing roles, where the risk of bias becomes exponential.

Why This Matters Now

Healthcare systems are under constant pressure to provide higher-quality care on tighter and tighter budgets. Unfortunately, cost-cutting and finding efficiencies is an inevitable part of that equation.

Until modern LLM process agents, that’s meant putting more work on fewer already-overworked staff members in a system with a ton of burnout. Now, with AI, hospitals are seeing a new path forward: streamlining their operations, unburdening their staff and focusing on patient care.

The University of Minnesota study is a reminder that even widely adopted tools can carry hidden assumptions and uneven performance. That is not a reason to avoid AI. It is a reason to be precise about where and how it is deployed.
